Does your webhost allow you to have php and sql? If so when you log into your webhost, see if you have a sql. If so click on SQL Administrator and create a database. (If you have already created a database, you need to plug in the name of the database and your admin user name and password in order for efiction to work).

If you're configuring your own server, I'm assuming you're using Linux and some distro of it? Or a Windows server?
Take a look at the MySQL Manual http://dev.mysql.com/doc/refman/5.1/en/installing.html

(Side note, SQL is different from MySQL)

Can you please give us more details so that we can help you further? Is this is local install, or a web server?

The server I am using allows php and my sql.
I use 110mb.com
It has mycql but need to pay so that to be installed and I don't have credit card!
Do you know if I can add it otherwise?

Looking at the features, SQL databases is a pay feature on 110mb. If you are wanting to use a free site that allows both PHP and SQL there are many webhosts out there. But Freehostia seems to be one of the easier ones.

I offer free web hosting, you can take a look at this thread, if you're interested.

You can't install MySQL on a free host because you need SSH access, and free hosts don't generally give SSH access as it can lead to users hacking or screwing up the server when given such privileges. There are tons of free hosts.
