I'm running a very old version of efiction (1.1), and I'm wondering about the best way to update. I assume I'd have to update to version 2 and then update again to 3? I did look at the directions for upgrading from version 1 to 2 and it seemed a little daunting. It's been many years since I've touched the installation, except for modifying the tpl files to update the style, so I have no recollection of whether I changed anything major back in the day. The script seems quite sturdy (have never had any issues with it), so my main reasons for upgrading would be: a) to have it running on more modern code, and b) to hopefully have a way to prevent spam regs. Secondarily, if the newest version had an easy way to mass delete all the spam registrations that have accrued over the years, that would be very nice.
I guess what I'm asking is, is it worth it to update? Or should I just keep my old version and try to manually delete the zillions of spam registrations?

You can go from version 1.1 directly to version 3, it's in the install script to handle it. I would recommend doing it if only for the security enhancements.
The new version will not be able to help you with the spam. The as-yet-unpublished version in the works may. But again, your site currently has security vulnerabilities.
