I have the latest version of efiction so following the readme that came with the block installed it correctly followed by activating it in the admin block options yet it still doesn't show up. Any ideas on this?

I can't get your site to load up, so this is just a guess. Did you add the block to your skins? It won't show up otherwise.
1. Turn off the Debug feature
2. If you have Shoutbox turned on, you still have to add it to your skin. Go to your skins folder and it will either be in the header.tpl or index.tpl default.tpl depending on who made the skin and add '{shoutbox}' '{shoutbox_content}' (without the quote marks) where you want it to appear.
You will notice that in your CSSZen skin, the shoutbox appears:
