It's important to note that I also had this problem in v 3.1. I was hoping the upgrade would fix it. Not really sure if I need to CHMOD the challenges.php file or this is something I need to do in the admin control panel.

Install the module. Log in as an admin and then go to:
http://library.iroh.org/modules/challenges/install.php

Uh, where do you find that in the Admin CP? I installed the other things from there.

Challenges module isn't found in the Admin Control Panel. You literally have to type in [your url]/module/challenges/install.php (Other modules you might download like the Recommendations module or the Word Display modules are the same way). There is no "module" section in the Admin Control panel. Mind you once you've installed the modules there is a link for them in the control panel but not before.
