This is something mysterious that I've never encountered before on my primary eFic installation ( http://drfa.seventh-star.net), which is currently running 3.5, using PHP 5.3.1.
When I go to Manage one of my own stories that's already validated and add something into each of the chapters' footnotes, I'll occasionally see this:

at the very top of the page. In other words, the header doesn't load where it's supposed to.
On some chapters, it appears right after the image that I've put in the footer of every endnote of my single fic's chapters. (That is, I've only done this for one fic, but for every chapter in the fic. Different chapters produce different results, but I haven't seen a pattern as to which chapters produce what problem, though they're all similar to this one: loading the page in a jumbled order.) On other chapters, the contents of the fic itself will appear at the top, followed by the site header and menu, then the "Your story has been edited" notice and the links to edit the rest of the chapters (the Manage Story page).
The HTML I'm adding is just simple linking code with an image code-- all allowed HTML. It's from Fanworks Finder, if that helps clarify things. It's plain ol' static HTML, not JavaScript or anything. When I check on the page (as if I were a visitor viewing the chapter), everything looks fine, so this only appears when I edit chapters. Because of this, it's not a huge problem, but I was wondering if it could be indicative of something wrong with either that I'm doing or with the latest update.
Again, because I've only noticed this with my one fic, I don't have a test account with a fake multi-chapter fic to test this out with. The Fanworks Finder code varies depending on the fic in question, too, and I can't create a fake fic listing on there just to get dummy code.
So, is it the code I'm injecting, something about the chapters themselves (because SOME chapters don't have this problem at all, and look fine when I go to edit them, and after I edit them and click "Add Story"), or something about 3.5? It's not like this is causing any huge problem to me, my fic/chapters, or my site that I can tell, but I'm curious. I also don't know if this has/would happen(ed) to other users who might want to edit their multi-chapter fics, or verify that edits have gone into effect (whether or not they're HTML edits).

Sounds like probably the code you're adding. Make sure your tags are closed and you might try turning of tinyMCE if you have it on. It may not be recognizing your tags as valid or trying to change them according to it's cleanup function.

The only thing that changes from fic to fic (note: NOT chapter to chapter) is the LISTINGID. I make sure to go into TinyMCE's "HTML Code Editor" when adding this, obviously, and it seems to display just fine, but it's only with some chapters that I get this problem of the page being mangled after adding this. Since all of the above is allowed code, and the image still shows up fine on the actual page when viewed normally (as if I were a reader of the fic, not the fic author or an admin editing it), that's why I was curious about the jumbling happening.

Do you have <img> and <a> in your allowed tags in your settings? That will also strip these out.
Catching up here. Did you have debug on? Looking at that again it looks like the debug comments got closed prematurely.
